Add these entries at the beginning:

display_errors = On
error_reporting = E_ALL
error_log = /var/log/php-errors
log_errors = On

This is about the maximum of error information, it will be
displayed and written to the log file.

You can easily look up the keywords in the web.

Then run your script, and see what the errors are.
