Is there anything more poorly documented than PHPUnit?
http://www.phpunit.de/manual/current/en/index.html

Could someone please tell me how to run a simple test
http://www.phpunit.de/manual/current/en/writing-tests-for-phpunit.html

where do I save example php block?
what filename do I give it?

does the php need to include or require any scripts
I've seen documentation saying to require PHPUnit.php
I've seen documentation saying to require Framework.php
neither (exists)

Did you try asking in the PHP unit support? Looks like they have an IRC
channel.

When dealing with a product, the best place to ask for help is from the
people who developed that product.
