One of my other hosts just got hit by a web defacing worm. The only files of mine that were affected were those who had o+w permission set.

I went into my main dedicated server and saw that all the fud files are set with that permission! Is this correct? Can I remove with o-w on all the fud files, or are these permissions set for a reason?

The forum needs to have the permission to write to it's own files for the purposes of upgrades and theme files during theme rebuild.

I'm a little confused by the concept of updating itself. If I have the owner as apache:apache and set the owner and group permissions as write, but remove the write permission from world, will this work?

The forum by default sets permissions on the files of 0600 and 0700 for directories. If you files are chmoded 666 it means that the forum was "unlocked" via the admin control panel or an individual file's permissions were changed via the file manager.
