I banned a user a while ago. They keep coming back even with the same IP address. How do I ban this person permanently at LEAST from using the same IP address over and over again?

You can ban them by IP, via admin IP filter control panel. You may also want to enable to so called "Bust'A'Punk" option (main admin settings panel) that will use additional means to block banned users from comming back.
