What settings/configuration is necessary for the system to send out the activation email? I am checking my spam folders, and am tailing mbox files, and feel very confident that the forum isn't sending out the activation email.
Where would you start troubleshooting this?

Well, you can check your mail logs on your server, those typically keep track of sent messages. Alternatively you can change PHP's sendmail_path to a shell script that would simply write to file when its called.
